Procedure for Requesting Tuition Assistance

1. Choose a college transfer course(s) based on your Plan of Study. (more info)
2. Complete the Community College Course Approval Form.
3. Mail the following as a complete packet to the address below.

• Completed Community College Course Approval Form
• Plan of Study (more info)
• Most recent pay stub (more info)
• A copy of proof of completing the FAFSA (for clients without a 4 year degree)
